Features

• Online questionnaires

  We implement your questionnaires as online forms, optimised for smartphone, tablet and PC and, if required, in German, French and English. Scales, yes/no questions, text fields and other question types make filling in easy. After submitting, patients can download a copy as a PDF.

• Automatic dispatch

  You decide when each questionnaire is sent, e.g. a feedback questionnaire one day after the procedure with one reminder. MonitOP sends it by email or text message with a personal link.

• Inbox

  Completed forms arrive in the inbox. You see at a glance which forms are new, which are already assigned to a case and which have a warning.

• Assign and archive

  Thanks to the personal link, a form is already assigned to the right case; all others are assigned in a few clicks. Forms without anything conspicuous can be archived automatically.

• Warnings

  You define which answers trigger a warning, e.g. when a patient is dissatisfied with their stay. Before such a form is archived, your team documents in a comment how it responded.

• Forms in the case

  Directly in the case you see which forms the patient has completed and which are scheduled. Send a form manually, fill it in together with the patient on site or unsubscribe the patient.

• Search and analyse forms

  The search also finds archived forms. Together with the Dashboards module you see the Net Promoter Score, the response rate and the average of each question.
